I give permission for my child(ren) to participate in all aspects of the VBS camp program at First Presbyterian Church. I understand that every effort will be made to contact me and the emergency contact if my child(ren) needs emergency medical treatment. By signing below I acknowledge that I release the First Presbyterian Church of Spirit Lake from liability and legal action stemming from my own actions, or my child(ren)’s behavior, injury and/or activity during the event. I authorize medical personnel, the VBS directors, or First Presbyterian Church pastors to secure any medical or emergency treatment as deemed necessary for my child(ren). I also give permission for any picture or video taken of my child(ren) to be used by First Presbyterian Church in church or online.

For 5th-6th Grade Students Only

I recognize that the 5th-6th grade VBS program includes activities that happen in a variety of sites in the Spirit Lake surrounding area. By signing below, the signed guardian gives permission to First Presbyterian Church to include their 5th-6th student(s) for all activities during VBS(Vacation Bible School) thatincludes travel to and from youth events leaving from the church premises. This will cover all events on one permission form during this time, instead of having multiple forms to sign for each activity. Unless previously arranged by me, I understand that he/she will be driven by a responsible adult or professional transportation provider chosen by the church. This document gives permission for my child to leave the church premises to participate in activities planned through First Presbyterian Church’s VBS program.
